IIS configuration for mixed mode authentication

What configuration of IIS work with example of mixed mode authentication (single sign on):

There are 5 options:
- Anonymous
- ASP.NET Impersonation
- Basic authentication
- Forms Authentication
- Windows authentication

How they should be set (what is enabled and what disabled)? Is there any other options to be set (e.g. Integrated Windows Authentication in IE, Automatic logon in Intranet zone in IE, etc...) in Internet Explorer?
How web.config should look like (e.g. authentication mode, impersonation, etc)?
How must I run the web site (as pass through or connect as some other user)? Connection string should be set with Integrated security or with username and password?

I have asp.net membership but want to authenticate user by its username from Windows.
